[…]\",\"content\":\"在過去半年來，收到太多私訊詢問我有關玻尿酸降解，也降解過各式各樣不同種的玻尿酸，所以決定寫這篇文。\\n先說如果遇到玻尿酸打失敗最好還請回原診所將解處理，說實在的沒有哪個醫師會想要幫別人擦屁股，降解是個非常吃力不討好的工作，要降解的客人通常情緒不是很好。自從遇到一些來降解的麻煩客人跟我鬧後，我突然就懂為什麼其他診所都不接降解了XD\\n最需要玻尿酸降解的人通常是[淚溝毛毛蟲或](https://goldiapple.com/tear-through-injection-failure/)是玻尿酸變成納美鼻，如果我在做鼻雕前或是幫他補淚溝玻尿酸前我一定會先降解處理掉才能再補新的玻尿酸或線材。\\n大部分人聽到打降解都巨害怕，到底降解酶是啥？其實人體本身就有降解酶，所以人體天然的玻尿酸，本來就會隨著老化，被自己產生的降解酶分解。\\n降解酶相當自然：由精子頭部提煉，讓你不會更凹。\\n直接講答案:六種情況建議打降解酶——淚溝玻尿酸毛毛蟲、玻尿酸山根鼻子變寬、法令紋玻尿酸位移、下巴玻尿酸太尖、額頭玻尿酸凹凸不平，以及全臉玻尿酸饅化或移位需要全臉Reset。\\n一般坊間降解費用有些用降解玻尿酸的cc數，有些用部位，費用通常落在5,000~15,000，視降解的劑量多寡而定。\\n降解酶的原理，就是精子想要捅破卵子的瞬間，會分泌類似降解酶的物質來進入卵子，因此降解酶的製成就是用牛羊的睪丸提煉而成。由於製成不易，小小一瓶拇指大的[降解酶其實超貴](http://www.tmias.org.tw/news/content.php?id=40)。\\n大部分診所不會想沒事搞事接降解客人，也造成大眾對玻尿酸降解酶的訊息取得較困難，通常如果原診所不願意處理，大醫院的皮膚科如台大醫院、長庚等醫美中心其實都是可以協助降解的。\\n什麼情況建議應該施打玻尿酸降解酶呢？\\n1. 淚溝玻尿酸毛毛蟲\\n淚溝毛毛蟲如果沒有降解，幾乎很難靠自體代謝就消失。因為打錯層次代謝極慢，我遇過很多超過五年以上都沒有消掉。如果遇到毛毛蟲，最佳解還是直接打降解溶掉。\\n2. 玻尿酸山根鼻子變寬\\n玻尿酸山根如果出現變寬的情形，一定要先打降解後，才能再補新的玻尿酸，否則很有可能越打越寬。\\n我印象很深，以前一個客人想要補山根，他上次施打的山根玻尿酸已經將近十年前，我心想應該已經什麼都不剩了，結果鈍針進去就像撞球桿推球一樣，我親眼看到一球不明物體被鈍針推到山根頂端，我只能趕快把針抽出來先把那球十多年來的玻尿酸殘留降解。\\n3. 全臉玻尿酸饅化或移位 → 你需要全臉降解重新Reset！\\n玻尿酸移位最常發生在施打的層次不對，最常出現在法令紋、下巴。\\n如果有位移的狀況不要太擔心，在位移的玻尿酸打一點降解就可以輕鬆消掉。\\n或是如果你打的玻尿酸劑型錯誤，小分子區打大分子，或是大分子區打太軟的玻尿酸，都建議先降解本來的玻尿酸，才不會補新的玻尿酸時變得凹凸不平。\\n打降解酶會不會變更凹？\\n首先要說打完降解幾乎一定會瘀青一週以上，因為降解會溶掉一些小微血管，基本上降解完是注定會有非常明顯的瘀青，所以如果妳是很在意瘀青的拜託請自己預留可以瘀青的時間再去降解。\\n再來就是大家最關心的議題，我以前都會跟客人說就是凹回你打之前的樣子，但我發現客人認知的凹跟我認知的凹是有落差的，大部分客人不敢相信自己打完降解後是長這個樣子，所以我現在一律都告訴大家，打完後就是會凹、會凹、一定會凹，但其實那就是你沒打前的樣子（不然你當年沒淚溝的話也不會沒事去補玻尿酸呀）。\\n降解酶雖然會稍微降掉一些人體的玻尿酸，但你自體還是會持續做出玻尿酸，不過除非你再補新的玻尿酸，不然你都不可能回到當初那個沒淚溝的自己，再加上許多人的毛毛蟲都經歷了數年，你的實際模樣早就比當年更凹。\\n要是全部消完後無法接受原本的自己，官方建議隔 48 小時左右可以再補新的玻尿酸，因為一般降解超過24小時已經幾乎沒有作用了。\\n玻尿酸降解有什麼的副作用？\\n一、凸起的皮鬆，你可以想像毛毛蟲就像一個胎兒一直在你的皮下，打完降解就像突然把懷胎十月的胎兒生出來，你的肚皮不會馬上回到原本生產前的緊度，需要時間慢慢貼回去，所以剛打完降解眼周的皮有點鬆鬆的正常。\\n二、尖針降解不乾淨而凹凸不平。\\n如果使用暴力降解法，看到哪裡凸起就用尖針戳下去，這樣除了會痛到如同地獄走一回，還很容易降不乾淨變得凹凸不平，還可能因為打不到正確的層次而降解到其他地方的玻尿酸。\\n最佳降解方法，是先照超音波看玻尿酸的層次，再用鈍針直接戳進玻尿酸的位置小劑量溶掉。\\n一次降不乾淨非常正常，尤其是施打多年的玻尿酸都已經跟組織融合非常難降解，甚至有些品牌的玻尿酸本身就很難降解，這種情況要求醫師要一次降解乾淨簡直強人所難，如果降解後局部還有凸起就再降解一次就好。\\n建議大家盡量玻尿酸失敗後兩個月內處理，因為玻尿酸會逐漸跟組織融合，越晚會越難看出玻尿酸的精準位置。\\n什麼東西無法降解？\\n玻尿酸就算失敗了不要太擔心，最糟都還有降解酶可以救。但如果要打童顏針、[精靈針](https://goldiapple.com/what-is-aesthefill/)或洢蓮絲的各位，就無法使用降解酶處理。\\n當然這些產品很持久我承認，但投資醫美有賺有賠，請詳閱公開聲明書 種常見狀況\\n不少客人都會很糾結到底要不要施打降解酶，覺得把花大錢打的玻尿酸溶掉很可惜？大部分原診所都會跟你說：「沒關係啦，玻尿酸一年就會自然分解掉。」\\n玻尿酸確實會代謝，但**「會代謝」跟「會消失得乾乾淨淨」是兩件完全不同的事**。\\n很多人出現玻尿酸延遲性過敏，拖越久反而出現玻尿酸結節硬塊，到時降解酶也於是無補，遇到以下幾種情況，我會建議狠下心打降解酶\\n降解酶多快見效？哪些情況很難一次降乾淨\\n降解酶打完後可以馬上把玻尿酸消除，效果立竿見影的快。\\n降解酶一打進去後就開始作用了，**前 30 分鐘大概就會溶掉五、六成以上**，接下來幾個小時之內，可以把該部位的玻尿酸幾乎溶解乾淨。\\n玻尿酸溶掉後會有變成液體狀，所以眼下還是會腫腫的，大概抓 **3-4天完全代謝後就是降解後的狀態**，如果降解完想要重新補玻尿酸，建議降解隔48小時候再補。\\n有些人降解一次就搞定，但也有少數人需要降解多次才會乾淨，這取決於玻尿酸是否完整**。**\\n如果是近一年才剛打進去的玻尿酸，玻尿酸形狀完整、邊界清楚，降解酶可以很容易就把整塊玻尿酸溶乾淨。\\n以下幾種情況很難一次降解\\n打降解酶最常見的 3 個迷思\\n講完原理，來講門診裡最常被問的三件事。這三個問題我幾乎每週都要回答一次，而且客人問的時候表情通常都很緊張。\\n迷思一：打完降解酶，會不會變得更凹？\\n我直接先講答案：**不會。**\\n有些客人會問我能不能只降解30%、50%，不想全部降解掉怕會凹，我只能誠實說我做不到，降解酶是會擴散的，打進去後這個部位的玻尿酸基本上就會溶解乾淨。\\n如果降解酶劑量抓太少，溶解不乾淨，反而後續更容易出現凹凸不平的狀況，所以我都會跟客人說，玻尿酸想保留就全留，不想要就全溶。\\n降解酶打完不會讓你變得更凹，最多就是讓你回到打玻尿酸之前的原狀。\\n它沒有辦法把你原本就有的組織拿走，所以它不可能讓你變得比原本更凹，如果是打類固醇就很容易變得比原本更凹，所以類固醇不建議輕易使用。\\n迷思二：降解酶會不會把我自己的玻尿酸也溶掉？\\n這題的答案比較有趣，「會，但不影響大局」。\\n降解酶不認主，它不會判斷哪部分玻尿酸是外來注射的、哪部分是你自體的玻尿酸，反正只要是玻尿酸它都溶掉。\\n所以當我把降解酶打進淚溝時，它不僅溶掉毛毛蟲，確實也會把你自己的玻尿酸一起溶掉。\\n但由於自體玻尿酸佔比非常少，所以就算溶掉也不會對外觀有顯著差異。\\n舉例來說，打完淚溝玻尿酸後，你自體的玻尿酸大概佔百分之一都不到。我們就算它有百分之一好了，那另外的百分之九十九全部都是外來的。所以就算打了玻尿酸降解酶後，你實際上損失的是那百分之一。\\n而這百分之一還會回來，自體組織大概三到四天左右會再把它補回去，外觀上幾乎不會有任何差別，因此不要糾結失去的這一點自體的玻尿酸就不敢去處理失敗的玻尿酸。\\n迷思三：打完降解酶會變更醜嗎？\\n這題我要講得比較直白一點，可能有點忠言逆耳，但我覺得講清楚對你比較好。\\n常常有客人問我：「歐醫師，那我降解完假設變很醜怎麼辦？是不是降解酶害的？」\\n我的答案是：降解酶只是讓你回到原狀，如果你覺得打完降解酶變醜，很抱歉，**你可能已經很久沒有看過自己的原狀了**。\\n尤其最多人降解完淚溝毛毛蟲後會問我眼下怎麼還有一包凸起，那就是你原生的眼袋，假設你3～5前打了淚溝，這幾年間你的臉持續在老化，並且眼下脂肪會持續堆積，除了原本的淚溝毛毛蟲外，眼袋也日漸茁壯，所以當你覺得眼下凸起到受不了時，某方面也是因為眼袋變大很多。\\n如果降解後你發現自己有眼袋，建議不要再回去重補玻尿酸，可以去諮詢眼袋手術把眼袋問題從根本解決。\\n同樣的不少人去除饅化後會覺得皮膚鬆弛，畢竟大量的玻尿酸就像你的臉懷胎數年，一下子把玻尿酸的體積都溶掉，就好像孩子突然生出來一樣，皮膚會出現鬆垂，後續可以搭配電音波或埋線拉提治療來恢復皮膚緊緻度。\\n所以請不要說「我打完降解就變得好糟糕、好憔悴」。降解酶沒有讓你憔悴，它只是把遮羞布拿掉，**讓你看到最原始真實的自己**。\\n恢復原狀後你才知道下一步真正該處理的是什麼問題。\\n降解酶施打方式：3 種玻尿酸類型與降解方法\\n前面鋪陳那麼久，現在進入最實際的部分，玻尿酸到底該如何降解？很多人以為打降解很簡單，隨便扎幾針就可以溶掉，不同的玻尿酸其實處理方式都不相同。\\n這也是為什麼打降解酶最好需要搭配超音波輔助，透過超音波顯影看清玻尿酸分佈狀態，才能更精準的施打降解酶。\\n類型一：單顆大團塊\\n這種類型是最容易降解的，通常是剛打不久的玻尿酸，邊界分明。\\n摸起來位置很明確，就是一兩顆，你自己在家用手指按都按得出來。不用笑、不用做表情，它就在那裡。\\n在超音波下，這種團塊非常明顯，通常當初施打的醫師是用尖針施打，一眼就看得出來這一顆特別大。\\n這種類型玻尿酸的處理方式是**精準爆破**，我們用一根尖針或鈍針直搗黃龍，戳到玻尿酸的正中心，注入降解酶，讓它從內部整個炸開。\\n通常這類型只要把降解炸藥投放下去，馬上就可以摸到玻尿酸消失，很多客人打完坐起來就會發現玻尿酸已經溶乾淨了。\\n類型二：分散型——補了又補的那種\\n這種是我覺得最麻煩的一種，尤其玻尿酸饅化大部分屬於這類型，客人經年累月補了很多次，甚至有時玻尿酸，有時膠原蛋白增生劑，在超音波下就像一鍋大雜燴。\\n在超音波下，玻尿酸散佈整個畫面，裡是**很多一球一球的小球**，每一球都小小的，但量加起來其實非常可觀。\\n最麻煩的不是量，是**散**。\\n這種類型不可能用尖針去降解，這麼多玻尿酸顆粒要戳幾百次，戳到整張臉都瘀青也戳不完。\\n所以做法完全不一樣。我們會用一根鈍針直接從一個入口灌進去，進到那個層次之後，開始大範圍地施打降解酶，並且會用**濃度比較高**的降解酶，讓它在整個區域裡作用，而不是點對點。就好像丟一顆原子彈下去，把這個區域的玻尿酸都轟掉。\\n這種打法可以讓分散四處的玻尿酸消掉，但缺點就是一次可能沒辦法完全根除所有玻尿酸，如果打完一次降解覺得還不夠，可以再進行第二次降解。\\n類型三：根本不是玻尿酸——洢蓮絲變成「假骨頭」\\n這種事最令人沮喪的，降解酶只能溶玻尿酸，其他材質通通沒救！\\n客人來的時候通常很沮喪，因為那塊硬塊他每天都摸得到，而且已經摸了很久。他以為那是玻尿酸沒溶掉，跑了好幾間診所打降解酶，一點反應都沒有。\\n原因很簡單：**那根本不是玻尿酸。**\\n如果是打膠原增生劑，比如說洢蓮絲（Ellansé）、童顏針、晶亮瓷等。它們的質地非常非常硬，硬到骨頭的程度。更麻煩的是，它有時候會被打在半空中——在一個本來不應該有骨頭的位置，長出一整塊很像骨頭的東西。你可以理解成，那是一塊**假的新骨頭**。\\n在超音波底下這件事非常明顯。洢蓮絲本身其實不是黑色的，它是一種很特別的白。但如果你看到那一整塊是全黑的，原因不是它像玻尿酸那樣呈現黑色，而是——**超音波根本穿不過去**。\\n超音波撞到那個硬度就直接反彈了，下面掃不到任何東西，沒有訊號，所以顯示成一片黑。\\n這種狀況，降解酶打下去是不會有任何作用的。降解酶只認玻尿酸。\\n那能不能處理？不好處理，但也不是不能處理。\\n膠原蛋白增生劑目前主要靠手術刮除，針劑只能打低劑量**類固醇**，用尖針、少量地打一點點。它沒有辦法像降解酶那樣立竿見影，這點要先有心理準備。通常打完隔兩週左右，那一塊會稍微小一點；下個月再打第二次，又會再小一點。\\n有些客人這樣要來打個六七次，甚至十次以上，才會感受到硬塊明顯變小。\\n這是一個需要耐心的過程，我不會跟你保證幾次會好。\\n三種類型對照表\\n打降解酶會很痛嗎？\\n會問這題的人通常已經決定要處理了，只是在心理建設。我把實際狀況講清楚。\\n打淚溝的那個區域，附近有一條血管叫做**眶下動脈**。而在這條動脈旁邊，還走著另外一個東西——**眶下神經**。\\n這條神經超音波是掃不太到的，因為它真的太細了。\\n但你感覺得到。如果鈍針就這樣直接進去裸刮，撞到這條神經的時候會**非常非常痛**，痛到整個臉會抖一下。這不是誇飾，是真的會彈起來。\\n所以我的做法是，在開始之前先做一個**神經阻斷**。\\n我們會先注射一點點麻藥，打到那條神經附近。麻藥上來之後，整個中臉這一區會全部麻掉。接下來鈍針才慢慢地進去，過程中再補刷一點很稀釋的麻藥。\\n我自己操作的時候會偏好用無痛的配方來打這個麻藥，讓打麻藥這個動作本身也不要變成另一個折磨。\\n為什麼我一直說要趁早處理？\\n這一段是整篇文章我最想讓你看到的。\\n前面提過，玻尿酸不會自己消失。但更精確的說法是：**它不只不會消失，它還會隨著時間變得越來越難處理。**\\n原因有兩個。\\n第一個是**散開**。玻尿酸在組織裡待久了，會慢慢跟周圍的組織融合，本來集中的一塊會越來越散。散掉之後，降解酶要作用的面積變大、濃度被稀釋，效率自然就下降。\\n第二個是**包起來**。你的身體會把它當成外來物，組織會分泌一些結締組織，慢慢在外面織出一層包膜。淚溝、毛毛蟲這種位置又特別容易被旁邊的肌肉一直擠壓、一直擠壓，包膜就越來越厚，裡面的東西越來越硬、越來越緻密。\\n等到這層膜成形，降解酶就很難接觸到裡面的玻尿酸了。\\n這個差別在超音波底下看得非常清楚。一個半月前打的玻尿酸，我可以掃得清清楚楚，邊界很透明，一看就知道在哪裡、有多少。但打了十幾年的，畫面上就是**整個像煙霧繚繞一樣**——你知道它在那裡，但你說不出來它的界線在哪。\\n我要特別澄清一件事：**打了十幾年、二十年的並不是溶不掉。** 只是相對於剛打不久的，它沒有那麼明確、那麼乾脆，需要的降解量會多很多。我最誇張的時候，一天曾經打超過十支降解。\\n所以如果你現在正在猶豫，我的建議很簡單：**早一年處理，跟晚五年處理，是完全不同難度的兩件事。** 而且成本也不同——需要的降解量、需要的次數都會增加。\\n那些拖了十幾年才來的客人，最後跟我說的話幾乎都一樣：「早知道就早點來了。」\\n常見問題
