{"status":200,"type":"application/json","body":"{\"code\":200,\"message\":\"操作成功\",\"data\":{\"id\":\"2112158853751746562\",\"ownerId\":\"5399814\",\"projectId\":\"493bc70e-546b-45d5-8f13-fd135e2230f1\",\"visibility\":\"PRIVATE\",\"title\":\"鼻子微整2種方式哪種更安全？玻尿酸隆鼻 vs. 埋線隆鼻優缺點論戰！\",\"slug\":\"hyaluronic-nose-augmentation\",\"summary\":\"玻尿酸隆鼻缺點有3個：多年後山根變寬（最常見）、血管栓塞失明（最危險）、交聯劑過敏硬塊。歐家宏醫師以每日打山根降解酶的經驗，解析喬雅登、V50、Vivacy、晶亮瓷、洢蓮絲打山根的缺點，以及山根變寬的正確補救方式。\",\"content\":\"『歐醫師，我山根想要變挺但不敢開刀，可以打山根玻尿酸嗎？』過去鼻雕還未普及時，鼻子微整的唯一方式只能打山根玻尿酸，但到底玻尿酸隆鼻缺點有哪些？跟埋線隆鼻該怎麼抉擇？\\n\\n**玻尿酸隆鼻缺點主要有 3 個：一是多年後山根變寬（最常見，喬雅登、Vivacy、V50 各品牌都難完全避免）、二是血管栓塞甚至失明（最危險）、三是交聯劑過敏產生硬塊。這也是為什麼我的診所大部分客人在做鼻子微整時，已經很少人選擇玻尿酸隆鼻，大部分是選擇埋線隆鼻（鼻雕）。**\\n\\n這篇文我會以每天幫客人打山根降解酶的第一線經驗，分享**3 大隆鼻方式比較**、**5 種填充物打山根的缺點**、**玻尿酸隆鼻後遺症**與**山根變寬的補救方式**，幫助你選出最適合自己的療程。\\n\\n玻尿酸隆鼻缺點重點速覽\\n\\n一、3大隆鼻方式：玻尿酸隆鼻、埋線隆鼻、手術隆鼻哪個好？\\n\\n**一句話總結：想大改鼻型（鼻頭、鼻翼、鼻小柱）最佳解直接考慮隆鼻手術；只想微調山根高度，玻尿酸隆鼻短期效果明顯但多年後容易變寬，埋線隆鼻增高幅度較小但自然、不會變寬。**\\n\\nDcard、ptt 上大家最常一提到隆鼻，就會看到微整派跟手術派吵起來，一派支持玻尿酸隆鼻、一派支持鼻雕，當然也有一派對於鼻子整形一律建議開刀。其實每個人的意見都有它正確的觀點，因為醫美沒有 100 分的方式，最主要還是這個方法適不適合你。\\n\\n不過對於不敢動隆鼻手術的人來說，以玻尿酸隆鼻或鼻雕的方法來微調山根高度，確實不需要承擔手術不可逆的代價，也不需要度過漫長的恢復期。以下我分別列出三種方式的優缺點供大家參考。\\n\\n1. 隆鼻手術：大改鼻型的唯一解，但風險與費用最高\\n\\n亞洲人大部分鼻子比較塌，鼻子微整能帶來的改變確實有很多侷限，例如鼻頭形狀、縮鼻翼這些確實是鼻子微整很難做到的。\\n\\n隆鼻手術優點除了維持時效久外，更能做到鼻子不同部位的修飾，包含鼻頭、鼻翼、鼻小柱，我有不少客人傳了一張韓星的鼻子給我，說想要靠鼻雕做成這樣，我跟她說你直接去隆鼻手術吧！如果是整個鼻型都要大改造、你心中有一個鼻子模板的，鼻雕、山根玻尿酸就別想了，直接手術才有可能做到煥然一新。\\n\\n當然，凡事都有缺點，你想享受換一個鼻子重生的美好果實，必然要付出較高的失敗風險，假體位移、多年後假體攣縮、形狀不如預期重修，這些都是手術隆鼻很容易遇到的後遺症，手術前一定要仔細評估自己是否有足夠的心理及經濟能力承擔，畢竟手術及萬一重修的費用都不便宜。\\n\\n2. 玻尿酸隆鼻：便宜、可逆，但產品特性註定會變寬\\n\\n玻尿酸隆鼻最大的優點就是用比手術便宜許多的價格、最短的恢復期換到一個挺鼻，不喜歡大不了就降解掉，可逆也是非常大的優點。\\n\\n玻尿酸隆鼻是將玻尿酸打在鼻子肌肉層下方的「深層脂肪」，靠近骨頭的位置，用玻尿酸做出一個像假體的東西，來墊高山根的高度。為了做出骨感類似假體的效果，必須選擇質地硬的大分子玻尿酸，例如喬雅登 Volux、Vivacy 玻尿酸、V50 玻尿酸這幾種，但就算選擇這麼硬的玻尿酸，第一年很好看沒錯，隨著玻尿酸跟組織融合，它很難一直跟假體一樣硬並且維持相同的形狀，隨著融合後變成液態，它最終就會流向鼻翼兩側最終變寬。\\n\\n我自己剛開業時也有推過山根玻尿酸，為什麼儘管可以打很硬的山根玻尿酸，我診所現在還是全面不做玻尿酸隆鼻了？主要是我作為少數有在接玻尿酸降解的醫師，我每天實在打太多山根玻尿酸降解，各種牌子無論喬雅登、Vivacy、V50、V18 我都降解過，這幾個劑型只是相對適合打山根，多年後照樣變寬。\\n\\n這意味著玻尿酸隆鼻雖然會有蜜月期，打完當下很好看，就算醫師技術很好，這個產品的特性終究是無法改變，很多人多年後都會後悔，遇到山根變寬問題來找我打山根降解酶。\\n\\n3. 埋線隆鼻：增高幅度較小，但自然且不會變寬\\n\\n[線雕隆鼻（鼻雕）](https://goldiapple.com/threadlifting-nose/)跟玻尿酸隆鼻原理基本上一樣，只是把玻尿酸換成線材放入，透過可吸收線材放到山根的位置來微調山根高度。\\n\\n它的缺點是山根增加的高度沒有玻尿酸明顯，因為線材體積很小，很難像玻尿酸液體狀增加很大的體積，但最大的優點是自然且美觀，以後不會遇到變寬的問題。\\n\\n下圖我的客人本身眼睛唇等五官都很漂亮，多年前打完山根後變寬讓她很苦惱，打完降解並且改成鼻雕後，整個五官立體度大幅升級，這就是為什麼我現在很少打山根玻尿酸的原因。\\n\\n二、玻尿酸隆鼻缺點：5 種常見填充物逐一分析\\n\\n**沒有任何填充物能完美避免山根問題：喬雅登、Vivacy 這類凝膠型玻尿酸多年後會變寬；V50 顆粒型最不易變寬但消退快；晶亮瓷、洢蓮絲雖然不易變寬，卻因為無法降解，失敗後最難處理。**\\n\\n1. 喬雅登、瑞斯朗玻尿酸隆鼻缺點：凝膠型代謝後流向兩側\\n\\n瑞斯朗女神、緹奧希、保柔緹、喬雅登玻尿酸都是典型的凝膠型玻尿酸，就算選擇最硬的分子，例如： 喬雅登Volux 施打山根玻尿酸，前面幾個月骨感美觀，但隨著玻尿酸慢慢與組織融合後，它的骨感程度會慢慢消失。\\n\\n凝膠型玻尿酸代謝後，凝膠的部分很容易跑到山根兩側，導致山根變寬，通常打完兩三年會想要降解融掉，因此我強烈不建議用凝膠型玻尿酸打山根。\\n\\n2. V50玻尿酸隆鼻缺點：最不易變寬，但消退非常快\\n\\n[V50玻尿酸](https://goldiapple.com/hyaluronic-acid-cleviel-v50/)算是山根玻尿酸中最不容易變寬的牌子了，在 V50 引進台灣後，台灣終於有『極硬』的玻尿酸，有效解決玻尿酸山根變寬，以及支撐度不夠的問題。\\n\\n但作為顆粒型玻尿酸，少了凝膠的體積，山根消掉的速度也比較快，（凝膠型玻尿酸可以靠凝膠在維持它的體積），V50玻尿酸打鼻子後，半年內通常會有一半以上的體積就已經消掉，反覆填充又會衍伸出山根變寬的問題。\\n\\n但是根據我試過幾乎市面上各種品牌最硬玻尿酸的經驗，V50是目前打山根的最佳解，除了維持度比較短外，後續副作用最小。\\n\\n3. Vivacy玻尿酸隆鼻缺點：夠硬，但仍是凝膠型\\n\\n作為歐美品牌頂級的超硬玻尿酸，它的硬度確實已經算是最適合打山根玻尿酸了，但它跟喬雅登有一樣的問題，都是凝膠型玻尿酸，多年後與組織融合後還是會變寬，我比較推薦這個牌子拿來打下巴玻尿酸。\\n\\n4. 晶亮瓷鼻子（再生針鼻子)缺點：無法降解，打過一次就回不了頭\\n\\n晶亮瓷跟玻尿酸比最大的優點是夠硬不易變形，施打後不容易出現山根變寬的現象，不少診所很愛用晶亮瓷打鼻子，但我從業至今堅持不打晶亮瓷山根，最大的原因還是它無法降解。\\n\\n加上山根能夠注射的位置狹窄，一旦打過一次晶亮瓷，後續要再打任何填充物非常困難。\\n\\n下圖案例是一個想找我降解山根的客人，超音波一照發現看起來不像玻尿酸，原來她打的是微晶瓷（晶亮瓷以前的名稱）而不是水微晶玻尿酸，她打完後長出的膠原蛋白長成一顆圓球，外觀看的到，但卻無法降解，只能手術清除膠原蛋白。\\n\\n我要強調晶亮瓷、洢蓮絲都是好東西，但是他們都不適合拿來打鼻子這種淺層又需要精雕細琢的部位，只要膠原蛋白長的位置差之毫釐，後果就會非常麻煩，這也是我絕對不使用任何一種膠原蛋白增生劑打山根的原因。\\n\\n延伸閱讀：[晶亮瓷適合打什麼部位呢？](https://goldiapple.com/what-is-radiesse/)\\n\\n5. 洢蓮絲鼻子缺點：增生失控又無法降解，最不建議\\n\\n洢蓮絲打山根是所有填充物中我最無法認同的，尤其 5-10 年前洢蓮絲剛興起時，一堆診所狂推洢蓮絲鼻子，我已經看過好多個客人打完洢蓮絲鼻子快崩潰的案例。\\n\\n洢蓮絲跟晶亮瓷一樣無法降解，它雖然很硬不容易變形，洢蓮絲打在夫妻宮、額頭這種大範圍凹陷的地方確實可以有很棒的支撐效果，但在山根這麼窄的空間，長得很好也會變得很寬、很硬的山根。不但大部分手術醫師不願意清創，如果失敗、形狀不滿意它還會持續非常非常久。\\n\\n下圖一位客人就是多年前打了洢蓮絲鼻子，除了山根還打了鼻樑，整個鼻樑都長出了很硬的膠原蛋白，她很後悔因為鼻樑看起來變很粗，這就是我一直強調的，鼻子是一個很需要精細的部位，如果膠原蛋白長稍微多一點，都會破壞精緻感。\\n\\n延伸閱讀：[洢蓮絲適合打哪些部位？](https://goldiapple.com/ellanse-price-and-introduction/)\\n\\n三、玻尿酸隆鼻後遺症及副作用\\n\\n**玻尿酸隆鼻除了高機率變寬外，最致命的後遺症是血管栓塞導致失明；其次是交聯劑過敏造成的硬塊與反覆腫脹。**\\n\\n1. 血管栓塞：所有注射部位中最危險\\n\\n每年幾乎都會發生玻尿酸隆鼻失明的慘案，很多人以為鼻雕聽起來很可怕，但其實每個醫師打玻尿酸隆鼻心理壓力才是最大的，就算我都已經只用鈍針施打了，還是沒辦法保證 0% 栓塞機率，淚溝我可以照超音波確定血管位置避開，但山根就算照了超音波，也非常難避免，山根位置有太多重要的動脈（延伸閱讀：[玻尿酸失明機率與 3 道安全防線](https://goldiapple.com/hyaluronic-acid-blindness/)）。\\n\\n如果你真的堅持要玻尿酸隆鼻，拜託你也絕對不要給新手練習，玻尿酸隆鼻看起來容易卻是所有部位中最危險的，今年我的醫師朋友群組中才看到有一家出事臉爛掉的照片，填充物的副作用絕對是比線雕多很多。\\n\\n2. 交聯劑過敏：山根硬塊、生病時腫起\\n\\n玻尿酸的交聯劑每個人多少都有可能過敏，我遇到不少過敏想要降解的客人通常是山根會摸到一球硬塊，生病或抵抗力不好時會整個腫起來。如果有這種狀況建議盡快[打降解酶](https://goldiapple.com/hyaluronic-acid-hyaluronidase/)融掉，否則拖得越久組織纖維化連降解都會非常困難。\\n\\n四、玻尿酸隆鼻山根變寬怎麼辦？\\n\\n**直接講答案：山根已有變寬跡象的話，建議先打降解酶融掉再重新施打，不要直接補打，因為補第二次山根玻尿酸 99% 會變更寬。真的想打玻尿酸隆鼻，單次劑量不要超過
