23,000（2026 年） 一、臉頰凹陷是什麼？ 一句話總結：臉頰凹陷就是顴骨下方出現的凹洞，陽光下會在顴骨底下形成陰影，也就是俗稱的「頰凹」。 像我本人就是開業後操心的事太多，瘦了不少，膠原蛋白流失後出現了臉頰凹陷。 很多人除了臉頰凹陷，同時還有夫妻宮凹陷與蘋果肌凹陷，整個臉部線條會像波浪一樣不順，顴骨在視覺上就更凸了。 很多人以為顴骨寬只能靠顴骨內推手術，其實只要把夫妻宮凹陷和臉頰凹陷補起來，臉部線條視覺上就會柔和許多。 二、臉頰凹陷原因：3 種頰凹類型 一句話總結：頰凹分成骨骼型、膠原蛋白流失型、臉部下垂型三種，治療方式完全不同。大觀念還是老話一句——流失（凹）靠填充，鬆垂靠拉提。 類型 成因 主要解法 骨骼型 顴骨位置天生較高，形成大範圍凹陷 填充 膠原蛋白流失型 很多歐美女星的頰凹都很明顯。歐美人種天生顴骨高，但下巴不後縮、山根又立體，顴骨高在整張臉上反而好看，甚至被視為個人特色；同樣的特徵放到亞洲人臉上，就會顯得無精打采、看起來很苦命。 骨骼型頰凹：顴骨天生較高，下方形成大範圍凹陷。 2. 膠原蛋白流失型：最常見，會讓人一下子老好幾歲 隨著年紀增長，膠原蛋白流失後兩側出現凹陷，通常會合併夫妻宮凹陷、蘋果肌凹陷，整體的臉部凹陷會讓視覺年齡急速增加。這一型透過玻尿酸或膠原蛋白增生劑，都可以讓臉恢復澎潤。 下圖這位客人原本臉部凹陷、看起來沒精神，施打薇貝拉後三個月回診，氣色好很多，整個回到年輕帥氣的樣子。 男性客人，膠原蛋白流失型頰凹，施打薇貝拉三個月後回診。 3. 臉部下垂型：只填充沒用，要先拉提 主要是蘋果肌下垂、嘴邊肉脂肪堆積，視覺上中間出現凹陷。這一型如果單純填充童顏針、精靈針，無法完全改善，需要搭配埋線拉提把嘴邊肉往上拉。 下圖這位客人原本很在意臉部線條的凹痕，經評估她並不是膠原蛋白流失，而是下垂問題——埋線之後頰凹就不明顯了，臉部也更緊實。 下垂型頰凹，以埋線拉提改善，未額外填充。 三、臉頰凹陷吃膠原蛋白有用嗎？不打針的改善方式 一句話總結：吃膠原蛋白沒用——所有「吃」的膠原蛋白到了小腸都會被分解成胺基酸，吃太多還會變成腹部脂肪。流失型頰凹治本要戒菸、別熬夜；下垂型則要靠音波或埋線拉提。 1. 流失型：戒菸、不熬夜才是治本 很多典型的流失型頰凹，最大原因是抽菸或時常熬夜。尼古丁和熬夜都會提高身體代謝，讓膠原蛋白加速流失。玻尿酸補完當然會立刻好很多，但如果一直熬夜，身體其他部位的膠原蛋白也在加速流失。 2. 臉頰凹陷吃什麼？只有過瘦的人吃才有用 有些人覺得自己頰凹是因為太瘦。如果真的是 BMI 過低，靠吃確實有效；但如果本身體脂率不低、BMI 正常甚至偏高，吃什麼都沒幫助。 那吃膠原蛋白總可以吧？很可惜，所有吃進去的膠原蛋白一到小腸就會被分解成胺基酸，甚至吃太多還會被儲存成腹部脂肪——頰凹沒改善，反而先變胖。 3. 下垂型：用音波把嘴邊肉「拉」回原位 如果臉部韌帶鬆弛、皮往下垂，會出現「臉頰凹陷、但嘴邊肉變明顯」的狀況。這時候是用音波拉提，把積在嘴邊的淺層脂肪拉回原本凹陷的位置，才比較能解決問題；更明顯的下垂則考慮埋線拉提。 四、臉頰凹陷打什麼？4 種填充材質優缺點比較 2. 臉頰凹陷童顏針：膠原長最多，但別一直補 優點：膠原蛋白增生效果最好，適合大面積頰凹 缺點：當下沒有立即填充效果，要等 2–3 個月長膠原；生長量很看體質；無法降解 劑量：輕度凹陷 1 瓶就夠，中度以上建議一邊一瓶，效果較顯著 要特別提醒：童顏針的膠原生長量是許多論文證實最多的，持續補很容易饅化。一瓶打下去，我比較建議 2–3 年都不需要再補。我聽過很多人周年慶一次買 5、6 瓶放著，每半年回去補一次——真的太多了，臉不需要長這麼多肉啦。 下圖是打一瓶童顏針、四個月後回診，頰凹已明顯改善，臉部線條也順很多。 童顏針一瓶，四個月後回診追蹤。 3. 臉頰凹陷薇貝拉／精靈針：結節率低的選擇 薇貝拉（魔法針）和精靈針的膠原蛋白增生成分基本上一樣（其實是同一位研發者跳槽到不同公司），所以放在一起講。 用新娘和伴娘來比喻：兩者的新娘都是聚雙旋乳酸（PDLLA），只是精靈針的伴娘是 CMC 凝膠，薇貝拉的伴娘是玻尿酸。 為什麼要加伴娘？主要是降低結節機率。增生劑本身是粉末、沒有體積，必須讓粉末均勻懸浮在組織裡，長出來才不會集中成一團變結節。但伴娘終究是伴娘——注射後一個月內就會退場，最後跟你過一輩子的是自己長出的膠原蛋白。 所以如果你在 Dcard、PTT 看到有人說「打精靈針、薇貝拉一個月就沒效了」，通常只是凝膠和玻尿酸的體積消失了。不要太失望，這類產品細水長流，精靈針約 頰凹較嚴重，薇貝拉兩瓶，施打一年後追蹤。 4. 自體脂肪：便宜，但變胖時會後悔 自體脂肪也可以改善頰凹，對臉非常凹的人來說，優點是相對便宜——畢竟打好幾瓶增生劑的價格不低。 最大的問題是未來體重增加時，臉會看起來胖很多。如果補的是玻尿酸，變胖時把玻尿酸溶掉，臉就能瘦回去，甚至不會再有頰凹；補脂之後變胖，那就真的只能這樣了。人過了中年，臉部脂肪堆積的速度會變非常快，很多人年輕時臉瘦、有頰凹，40 歲後臉突然開始變肉——我有不少客人變胖後蠻後悔補脂的。 5. 為什麼我不建議頰凹打洢蓮絲、晶亮瓷？ 因為這兩種材質特別硬。它們適合打在需要骨架支撐的額頭凹陷和夫妻宮凹陷，但臉頰是很柔軟的區域，做表情時很容易有僵硬感。 韓國玻尿酸，讓硬度更接近臉頰真實觸感，同時保留膠原生長的效果。 五、臉頰凹陷玻尿酸怎麼打？避免頰凹變頰凸的 3 個關鍵 一句話總結：打頰凹最怕「頰凹打成頰凸」。關鍵有三個——一定要打通韌帶、要打在咀嚼肌上方的頰脂肪墊、要把顴骨到下顎線修順。 1. 3 條韌帶把臉頰分成 4 區：一定要打通韌帶才能補 臉頰有 3 條大韌帶，把臉頰畫成 4 個區。不是每個人 4 區都凹，有些人甚至只凹一區——玻尿酸好就好在，想精準補哪個區就補哪個區。 如果醫師沒有把各區之間的韌帶打通，直接往臉頰打一堆玻尿酸，玻尿酸就會全部卡在其中一區，做表情時臉上就會凸起一塊。這是頰凹玻尿酸最常見的後遺症，也是「頰凹打成頰凸」的主要原因。（Ref: Hee-Jin Kim, Kyle K. 頰凹玻尿酸最怕針沒有打在這層。沒打到的話，玻尿酸沒辦法被固定，會漂浮在臉頰的空間裡，有點像太空飄浮的感覺。透過超音波，可以清楚看到頰脂肪墊與周圍構造的位置。 超音波下可清楚看到咀嚼肌（咬肌）上方的頰脂肪墊。 3. 顴骨→咀嚼肌→下顎線：三點連成一線才順 打頰凹玻尿酸一定要修「順」，也就是顴骨 → 咀嚼肌 → 下顎線，三點連成一線，線條才會好看。 我的做法是由耳屏軟骨（tragus）前 1 上的文章，都找不到「臉頰凹陷」和「音波拉提」的關聯；但很多客人看了 Dcard 都說打完音波會凹。主要原因有兩個： 音波把下垂的肉往上拉提後，視覺上原本的頰凹會更明顯 消脂探頭打的發數太多，臉瘦下去之後頰凹就出現了 2. 咀嚼肌肉毒：打到上半部就會凹 施打咀嚼肌肉毒時，千萬不要打到咀嚼肌上半部。咀嚼肌往上連到顴骨，打到上半部不僅對瘦小臉沒幫助，還會讓顴骨下方直接凹下去，造成術後頰凹。 3. 埋線拉提：拉太大力或線走進頰凹區就會更凹 埋線如果拉得太大力，確實可能讓頰凹更嚴重——法令紋拉不見了，頰凹卻更明顯。所以效果與副作用一定要取捨；埋線時也要避開頰凹區，不要讓線走進去。 本身就有一點頰凹、又想做埋線的人，建議搭配玻尿酸、童顏針或精靈針做複合式治療，才不會埋完線頰凹看起來更嚴重。詳細做法看：埋線拉提完整介紹。 臉頰凹陷常見問題 臉頰凹陷原因是什麼？ 主要分成骨骼型、膠原蛋白流失型、臉部下垂型三種。我診間 45 歲以下的客人大多是流失型，適合用玻尿酸或膠原蛋白增生劑填充；45 歲以上較常見下垂型（鬆弛型），建議改用音波或埋線拉提。 臉頰凹陷要打什麼材質？ 最適合的是玻尿酸、童顏針、薇貝拉、精靈針四種。局部小凹點選玻尿酸精準補；大面積凹陷先打膠原蛋白增生劑，剩下的小凹點再用玻尿酸收尾。我不建議頰凹打洢蓮絲或晶亮瓷，這兩種太硬，臉頰做表情容易僵。 臉頰凹陷玻尿酸要打幾 cc？ 依凹陷程度通常抓 2–4cc。一般局部凹陷 2cc 就能修飾線條；如果是暴瘦、大量流失造成的凹陷，玻尿酸會超過 3cc，這時可以考慮改打童顏針或精靈針 1–2 瓶。 打臉頰凹陷玻尿酸有後遺症嗎？ 最常見的是「頰凹打成頰凸」。臉頰有 3 條韌帶把臉頰分成 4 區，醫師如果沒有把韌帶打通就直接打，玻尿酸會卡在其中一區，做表情時就會凸起。所以打頰凹一定要打通韌帶，並打在咀嚼肌上方的頰脂肪墊。 打臉頰凹陷玻尿酸安全嗎？ 臉頰是臉部最安全的注射部位之一，沒有重要的動脈或血管，不太需要像淚溝那樣用超音波確認血管位置。不過超音波仍可以確認玻尿酸有沒有打在頰脂肪墊，避免玻尿酸沒被固定而漂浮。 臉頰凹陷補玻尿酸會痛嗎？ 不太會痛。頰脂肪墊本身沒什麼神經，加上用的是鈍針，現在的玻尿酸也多含麻藥，施打過程的舒適度比無麻藥的高出很多。 臉頰凹陷玻尿酸可以維持多久？ 約 1–1.5 年。以前玻尿酸交聯技術不成熟，大約一年就消；現在無論平價或高價品牌，大多能維持 1–1.5 年。第二次補打的劑量通常也不需要像第一次那麼多。 臉頰凹陷打玻尿酸多少錢？ 韓國蜂巢玻尿酸 2cc 17,000、喬雅登 2cc 27,000、瑞斯朗
